What Is This Service?

What Is Post-Hospitalization Transitional Care?

Post-hospitalization care is a structured, time-limited, nurse-supervised program that supports patients during the critical recovery period immediately following discharge from a hospital, skilled nursing facility, or rehabilitation center. It is one of the most clinically important phases of care — and one of the most frequently underserved.
At All Hands Home Health Services, our licensed nurses work directly with the discharging hospital team to receive a full handoff of the patient’s care plan. From day one at home, we manage medications, monitor vital signs, assess wound healing, educate the patient and family, and maintain active communication with the treating physician.
Our goal is simple: keep your loved one safe, comfortable, and out of the hospital during the 30-day window when readmission risk is at its highest — and to support a smooth, full recovery that extends well beyond that window.
Why the first 30 days matter: The transition home is the most dangerous phase of recovery. Medication errors, undetected complications, and inadequate follow-up are the leading causes of preventable readmissions — all of which skilled nursing care can directly prevent.
Hospital Care Plan Handoff
We coordinate directly with discharge teams to receive a full transfer of the patient’s hospital care plan before they leave.
Same-Day or Next-Day Start
We target first nursing visit within 24 hours of discharge — minimizing the unmonitored gap that leads to complications.
Medication Reconciliation
Complete review of all new and existing medications post-discharge to prevent dangerous errors, duplications, and missed doses.
Vital Sign Monitoring
Regular monitoring of blood pressure, oxygen saturation, temperature, pulse, and respiratory rate to catch deterioration early.
Family Education
We train family members on warning signs, medications, wound care, and when to seek emergency care at each visit.
Physician Communication
Direct, documented updates to the treating or primary physician after every nursing visit during the transitional period.

Medicare Readmission Rate

The Hospital Readmission Problem Is Preventable

Approximately 1 in 5 Medicare patients are readmitted to the hospital within 30 days of discharge. The most common causes — medication errors, unmonitored wound complications, and inadequate follow-up — are all preventable with skilled in-home nursing care during this critical window.
At All Hands Home Health, our post-discharge nursing visits are specifically designed to catch warning signs before they escalate, ensure medications are taken correctly, and maintain the communication loop between patient, family, and physician. Families who invest in transitional care consistently see fewer readmissions, faster recoveries, and significantly less post-discharge anxiety.

Why It Matters

What Happens When Post-Hospital Care Is Skipped

Returning home without a professional nursing plan puts recovering patients at serious risk. These are the most common and preventable complications we see.
30-Day Readmission
Unmonitored patients return to the hospital within 30 days at alarming rates — often for complications that a nursing visit would have caught and addressed early.
Medication Errors
Discharge prescriptions are often new, multiple, or complex. Without nurse oversight, missed doses, wrong timing, and dangerous combinations are common.
Wound Infections
Surgical sites and wounds that are not professionally monitored and dressed are vulnerable to infections that can become life-threatening rapidly.
Falls & Reinjury
Patients recovering from surgery or illness are at significantly elevated fall risk. Unassessed home environments and reduced strength lead to serious falls and setbacks.
Caregiver Burnout
Family members without professional support often take on medical responsibilities they are not trained for — leading to exhaustion, errors, and emotional distress.
Financial Consequences
A single avoidable readmission can cost a family thousands of dollars and weeks of additional recovery time — far exceeding the cost of preventive nursing care.
